【案例一】一般情况:患者,姚某某,男性,30岁,已婚,公司职员,大专文化。主诉:发作性心慌、气急、恐惧2个月。现病史:2个多月前,患者的表姐夫突发心脏病死亡,当时刚好当地新闻媒体报道了一位30多岁的青年教师在晚间突发心肌梗死死亡的消息。数天后,患者突然感紧张、害怕、心慌,自己搭脉搏达每分钟100次以上,持续约10分钟左右即消失。第二天患者到医院就诊,查心电图示:T波轻度改变。接诊医生告知患者,其心脏没有问题,心电图提示的T波改变也并不严重,只需要稍微对症处理一下就可以了,患者当时表示理解接受。一个月前,患者从上海回家,在汽车上又突然莫名其妙地恐惧、紧张,同时感心慌、胸闷、呼吸困难,感到“自己快要死了”。当时立刻在上海某医院急诊,查心电图示:窦性心动过速;T波轻度改变。给予对症处理后症状缓解。但其后患者又出现了多次类似发作,每次发作时均伴有心慌、心跳加快,每分钟达140~180次,呼吸急促,发作持续30~40分钟后缓解。在缓解期患者仍然表现忧虑发愁,害怕独处,工作时注意力不能集中,虽能坚持工作,但自感工作效率明显下降。夜眠差,主要表现为入睡困难。患者自感苦恼,故到当地心身医学科住院治疗。既往史:患者平素身体健康,无伤寒、肝炎等传染病史,无手术外伤史,无药物食物过敏史,无输血史,免疫接种史不详。个人史:生于母第1胎,母孕期正常,婴幼儿期发育良好,童年无不良遭遇。7岁上学,读书成绩可,人际关系可。25岁结婚,夫妻感情一般。育有一女,现年4岁,与患者关系好。患者无特殊兴趣爱好,不嗜烟酒。个性特征:温和、少语。家族史:两系三代否认有精神异常史。体格检查:T:36.5℃,BP:120/70 mmHg,神清,颈软,心、肺听诊(-),腹平软,肝脾肋下未触及。神经系统粗查未发现明显异常。辅助检查:胸片:未见明显异常;ECG:窦性心律,T波改变;EEG:正常范围脑电图;血常规:正常;尿常规:正常;肝肾功能:正常。心理量表测评:汉密尔顿焦虑量表(HAMA)总分:22分。请回答以下问题:试述该患者的诊断及诊断依据,还应与哪些疾病相鉴别?
