[事故经过] 1991年9月26日,中国铁道建筑总公司第十四工程局三处十一队在杭州市钱塘江二桥北引线配套工程某立交桥施工中,桥的第三孔右侧边梁倾覆,造成4人死亡,重伤1人,轻伤1人。 1991年8月21日,第三工程处十一队架梁组在架完某立交桥第三孔梁后,该队助理工程师殷某进行质量检查,发现该孔右侧边梁西头向外偏移5厘米。经队长和主管工程师同意,在8月31日给架梁组下达了调正该梁的通知书。9月10日,在架梁组长庞某指挥下,将该梁调整到位。9月24日,在立模组长的带领下,在该梁上的外侧立防撞墙模板。 9月25日,又进行了混凝土灌注。9月26日7时30分,立模组长带领本组7名工人在某立交桥第三孔右侧边梁上拆除防撞墙模板。在拆除固定模板的十根斜拉钢筋中的第九根时,该梁突然倾覆,2名工人随梁坠落,一人死亡,一人轻伤。此时,正在运送水泥的柴油翻斗车行至此梁右侧,陷入泥中,有三名工人帮助推车,梁体砸在翻斗车上,当场砸死 3人,砸伤1人。 [事故原因分析] 1.违章作业是造成此次事故的直接原因。9月10日19时,在架梁组长庞某的指挥下,将偏移的梁体调整到位后,庞即告诉电焊组长韦某把梁的连接钢板焊接上。但韦没及时焊接,而是将两毛梁上沿预留的钢筋扭动连接起来,作为临时加固,但以后忘了焊接。立模组在立模前,未检查梁的稳固情况就进行立模、灌注,致使拆除防撞墙模板时,造成该梁重心外移而倾覆。 2.检查制度不落实是造成此事故的重要原因。负责指挥施工的副队长王某没对上道工序检查就安排了立模灌注。负责技术指导的殷某在移梁后也没有对梁的稳定性进行检查。翻斗车进入第三孔梁右侧边梁下陷入泥中,由于无安全防护措施,造成事故扩大。 [对事故责任者的处理] 1.担任配属架梁组焊接桥面连接钢板任务的电焊组长韦某是此次重大事故的直接责任者,给予留用察看两年的处分。 2.副队长王某是此次事故的主要责任者,给予撤销副队长职务处分。 3.队长王某负责队的全面工作,对现场施工组织不严,对此次事故负有重要责任,给予行政记大过处分。 4.助理工程师殷某对移梁后的技术安全工作检查不细,立模组长李某在施工前没有按规定检查确认上一道工序是否完成,就盲目施工,对此次事故负有一定责任,分别给予行政警告处分。 [防止同类事故的措施] 1.牢固树立“安全第一、预防为主”的思想,增强安全意识。 2.加强安全教育和岗位技术培训,狠抓各项规章制度的落实。 3.强化基础工作,严格劳动纪律,加强对民工队伍的管理,严格按规定录用。 1.确定此次事故的性质。 2.简述事故应急预案的作用。
